Community Assessment Team

Emergency Response Team

The Emergency Response Team (ERT) is operational 24 hours a day, 7 days per week. ERT accepts referrals from all health and allied health professionals.

Social Work Team

This week day service works as a part of an MDT in assessing care needs, commissioning and reviewing services as well as offering advice and signposting patients and their relatives to appropriate resources.

The provision of statutory services is within Liverpool City Council’s Fair Access to Care criteria, who we work very closely with. LOOHS Socail Workers also undertake Safeguarding Vulnerable Adult investigations on behalf of LCC.

Therapy Team


LOOHS therapists are operational Monday – Friday with core working times between 08:30 and 17:00.

How to refer

What we do

The aim of the service is to support people to return home after a recent hospital admission at the earliest opportunity, to enable people to live safely and independently, and to prevent people from being admitted to hospital if possible.

The service delivers multidisciplinary assessments and patient centered rehabilitation/re-enablement to help people to recover from recent loss of function this care is offered in a variety of settings across Liverpool, such as Intermediate Care Hubs working in partnership with the Local Authority or BUPA, other nursing and residential care homes, and in the community in patient’s own homes.

The Therapy team consists of Occupational Therapists, Physiotherapists and Assistant Practitioners that are integrated with the nursing, social work and care elements of the Liverpool Out of Hospital Services. This enables the team to provide responsive, appropriate and holistic care and rehabilitation to Liverpool residents accessing the service.

LOOHS Therapy accepts referrals from all health and social work professionals.
