What We Do
The Community Heart Failure Team - Liverpool
The Community Heart Failure Team comprises of 5 heart failure nurses, and operates city wide. This is a specialist Heart Failure service geared towards the management of patients with a diagnosis of heart failure.
The aim of the team is to manage patients with a clinical diagnosis of heart failure, in order to optimise anti-heart failure therapies, improve quality of life, and reduce unnecessary admissions to hospital.
Patients can be reviewed in community heart failure clinics, and also at home if patients are housebound, or have deterioration in symptoms.
The Heart Failure Team will also provide education and supportive management to clinicians.
The Heart Failure Team - Sefton
The team operates across both North and South Sefton, and more recently West Lancashire.
The aim of the cardiac service is to provide care and support as needed to all patients with cardiac conditions.
These conditions include heart attack, valvular heart disease, coronary artery bypass graft, heart failure, cardiomyopathy, angioplasty, stents, ICD and biventricular pacemaker implants.
The Community Cardiac team consists of 5 Cardiac Specialist Nurses, 5 Community Cardiac Nurses, and 2 Community Cardiac Rehabilitation Nurses. There are also 2 Health Care Assistants who support the team to work across both North and South Sefton.

Liverpool Team Information
All referrals are received by fax (0151 296 7528) with confirmed diagnosis of LVSD based on ECHO result.
Copies of referral form/criteria have been forwarded to each GP practice and can also be downloaded here. Health professionals should complete all sections of the referral proforma, and fax to 0151 296 7528 with a copy of the most recent ECHO.
Sefton Team Information
Referrals are taken from GP, hospital consultant or cardiac nurse, or other health professionals.
We provide timely reviews at regular intervals, to give advice and information for patients over 18 years of age to assist with the ongoing management of their cardiac condition and optimisation of their medications. This is offered as clinic or home review as condition needs.

The Community Cardiac Rehabilitation Nurses also review patients after discharge following myocardial infarction, primary angioplasty, or surgical intervention such as coronary artery bypass grafting or valve replacements. They are offered supportive cardiac rehabilitation advice and information to assist recovery and promote a healthy lifestyle. This is offered as a telephone, clinic or home review immediately following hospital discharge.
