Community Matrons

What We Do

Community Matrons are highly experienced nurses who work closely with patients and their doctors, to plan and organise care.

Community Matrons are experienced, skilled nurses who are equipped in case management for patients who suffer with complex long term conditions and currently have a very high intensity use of health care. With special intensive help, these patients are able to remain at home longer and to have more choice about their health care.

The Community Matron will coordinate all the health and social care a patient needs, ensuring people get the right help at the right time. This can result in less duplication and fewer unnecessary trips to the hospital or to the GP.

Community Matrons are based across the Liverpool and provide care to patients within their own home and community setting.

Referrals

Access to the Service is through a GP, although referrals are considered from all Health and Social Care professionals.
