Social Inclusion Team

What We Do

Excluded groups within our population are the focus for the Social Inclusion Team’s work.

Our role is to create knowledge of health services in these groups and individuals, and encourage access to those services. The team’s experience, knowledge and skills integrate with a range of other professionals and agencies, to open up access to care services for vulnerable individuals and families.

The team’s activities are wide ranging and flexible to allow tailored approaches, but the main strands include:

  • Outreach work for people having difficulties accessing primary care services
  • A Link Worker service engaging with individuals
  • Signposting and advocacy work

The team is also pivotal in advising, guiding and training health professionals in culturally-sensitive service delivery, aided by representatives from the Arabic, Bangladeshi, Chinese, Irish, Polish, Pakistani, Russian, Somali, Yemeni, Gypsy & Travellers and the deaf communities.
