Community Falls Team

What We Do

Liverpool Community Falls Service

The Community Falls Team is a Multi-disciplinary team consisting of Physiotherapists, Occupational Therapists, and a part time Podiatrist, who are supported by Assistant Practitioner staff. We provide assessment and rehabilitation for adults who have fallen or who are at high risk of falling. We work with patients and their carers within their own home or in the local community.

The aim of this service is to provide an individual treatment plan using the Care Aims model, to restore independence and confidence and reduce hospital admission. This may include; an exercise programme to improve balance and strength, advice on how to complete everyday tasks in a way to make them easier and safer, and guidance on foot care and suitable footwear to help prevent falls.

Patients may attend an eight week Falls Prevention Programme at Venmore Community Care Centre which is run in conjunction with Liverpool City Council. This provides a group based exercise programme, discussion groups and individual therapy input. More information about the Falls Prevention Programme can be downloaded in the leaflet.

We are a Liverpool city wide service for those over 18 years of age with a Liverpool GP.

Referrals are accepted from health or social care professionals, the patient must consent to the referral. We do not accept referrals which request equipment only. Download the Liverpool referral form here.

Patients who have difficulty with their memory or retaining information may not be suited to our programme. Those referrals will be signposted to the other Therapy services within our organisation who we work closely with.
